LOCAL 6 GROUP HEALTH INSURANCE
Through an arrangement with French Cormany Insurance Services, Inc of Irvine, California, Local 6 is now able to offer self-pay, group health insurance to its members. This group health plan is not intended for members 65 and older. Members interested in obtaining rates on medigap insurance should contact the local, and we will put them directly in touch with our broker. The available plans are through Kaiser (HMO) and Health Net (both HMO and PPO). All of the plans offer guaranteed acceptance and coverage for pre-existing conditions (for the HMO plans - from the first day that the policy goes into effect; for the PPO plan - after a six month waiting period, unless you have had continuous coverage for the six months prior to enrollment). Your coverage will begin on the first day of the month following the month that we receive your enrollment forms and the first and last months’ premiums. To retain coverage through this program, you are required to keep current with your Local 6 membership dues, i.e. you cannot be suspended.

LOCAL 6 GROUP DENTAL INSURANCE
Effective December 1, 2009, Local 6 members can obtain dental insurance through CEA’s Delta Dental program. Members can choose either the PPO or HMO dental plan.
The Delta Dental PPO program includes: Perio, Endo and Oral Surgery coverage under Basic Services; Dental Implant Coverage; No waiting periods; Access to over 96,000 PPO offices nationwide and over 176,000 Delta Premier dentists; Visit any Delta Dental PPO or Premier dentist and receive greater cost savings, no balance billing and guaranteed quality of work.
The DeltaCare®USA DHMO program includes: Coverage for nearly 300 procedures; A 3rd Additional Cleaning, White Fillings, Bleaching, General Anesthesia, Orthodontic Takeover and Night Guards.

LOCAL 6 HEALTH INSURANCE FACTS
- Local 6 is now able to offer self-pay, group health insurance to its members. This is made possible through an arrangement with French Cormany Insurance Services, Inc. located in Irvine, California. This group health plan is not intended for members 65 and older. Members interested in obtaining rates on medigap insurance should contact the local, and we will put them directly in touch with our broker.
- The available plans are through Kaiser (HMO) and Health Net (both HMO and PPO).
- All of the plans offer guaranteed acceptance and coverage for pre-existing conditions (for the HMO plans - from the first day that the policy goes into effect; for the PPO plan - after a six month waiting period, unless you have had continuous coverage for the six months prior to enrollment).
- Members may join beginning the first of any month following the month that we receive enrollment forms and payment for the first and last months’ premiums. Members who drop off of the plan may rejoin, but they must wait until the next open enrollment period.
- Open enrollment for Kaiser is November 1; open enrollment for Health Net is January 1. These are also the months that rate increases are implemented.
- After your initial enrollment, Local 6 will send you a bill at the beginning of each month for the following month’s premium. Payment is due by the 10th of the month.
- Please note that rates for both Northern and Southern California are included in the information packet. The applicable rate is for your home address, and you must use medical facilities within 30 miles of your residence except for emergencies. Therefore, for most of our members, the rates for Northern California will apply.
- To retain coverage through this program, you are required to keep current with your Local 6 membership dues, i.e. you cannot be suspended.
